How to Enter: During the entry period visit Nashville Makeover on the Sponsor’s website https://smileonnashville.com/the-nashville-makeover-contest-2022/ follow the instructions to complete and electronically enter for your chance to win. You will be asked to provide basic contact information including but not limited to; Your name, address, phone number and email. All information fields must be completed for your entry to be accepted. All entries become the property of the Sponsor and none will be acknowledged or returned.
Limitations. Limit one entry per contestant. All entries must be submitted by the contestant, with no third party applications accepted. No nominations permitted and no sweepstakes entries please.
Drawing. The winner will be selected and notified within 14 days after the last entry day. Selection criterion is twofold: 1. Your written essay articulating how the Nashville Makeover will positively enhance and change your life. 2. Contestants will send 2 frontal facial pictures with their best natural smile. This is an initial diagnostic for digital smile design.
Consent. By entering this contest you give your express consent to Smile on Nashville, and its assignees, to record and use your persona, image and likeness in still and motion content for any and all marketing as Smile on Nashville sees fit.
